Description
Mixed Epithelial Stromal Tumor (MEST) Description
A Mixed Epithelial Stromal Tumor (MEST) is a rare and distinct type of benign tumor that affects the kidney. It is characterized by its biphasic composition, consisting of both epithelial and stromal components.
- Epithelial Component: The epithelial part of MEST is typically cystic or slit-like in architecture, lined with cuboidal to columnar epithelium. This component may exhibit hobnailing, papillary infoldings, or tufting, and can have a basal layer with two cell layers [10].
- Stromal Component: The stromal part of MEST is composed of spindle-shaped cells that mimic the appearance of normal kidney tissue [6]. This component can be solid or cystic in nature.
MESTs are relatively rare tumors that predominantly affect middle-aged or older females. They are usually well-circumscribed and located close to the hilum and renal pelvis, similar to multicystic nephroma [11].
Key Characteristics
- Biphasic composition of epithelial and stromal components
- Cystic or slit-like architecture in the epithelial component
- Spindle-shaped cells in the stromal component
- Typically affects middle-aged or older females
- Well-circumscribed and located close to the hilum and renal pelvis

Signs and Symptoms
Mixed epithelial and stromal tumor (MEST) can present with a variety of signs and symptoms, which may include:
- A palpable abdominal or flank mass [8]
- Flank pain [5, 8]
- Hematuria (blood in the urine) [7, 8]
- Urinary tract infections [5, 8]
- Abdominal mass [8]
It's worth noting that not all individuals with MEST will experience symptoms, and some may be asymptomatic. The clinical presentation of MEST can vary depending on the size and location of the tumor.
In rare cases, MEST can also present as a well-circumscribed multiseptate cystic mass with thin or thick septae and enhancing solid components on imaging [9]. However, this is not a common presentation and may require further investigation to confirm the diagnosis.
Additional Symptoms
- Urinary tract infections
- Hematuria (blood in the urine)
